REGISTER EACH INDIVIDUAL THAT NEEDS AN APPOINTMENT SEPARATELY AND WITH THE PATIENTS NAME.
DO NOT REGISTER ANY NAME MORE THAN ONCE!
Once we receive your registration someone will give you a call to schedule an appointment with us for one of our locations in March. Your appointment location will be determined during the pre-screening call.
A TICKET MAY BE SENT TO YOU BY EVENTBRITE – YOU WILL NOT NEED THIS FOR THE CLINIC. DO NOT PRINT IT!!!
